Child Health and Development Institute (CHDI)

CHDI places great emphasis on achieving sustainable improvements in primary and preventive health. As a result of this prevention focus, many of our efforts are concentrated on the early years of childhood. 

• Helping communities build links between health services and early childhood programs through the Promoting Health & Learning initiative
• Building a system of medical homes in Connecticut, so that children and families have an ongoing relationship with a primary care practice that meets all of their children’s health and developmental needs
• Ensuring that children and families are connected to the health and other services that they need and that this care is coordinated through their medical home
• Ensuring that the pediatric health care system in Connecticut is family-centered and responsive to families who are culturally diverse
