Grant Opportunity for Pediatric Practices
CHDI is pleased to announce a new grant opportunity for pediatric primary care practices. The grants are funded by the Children's Fund of Connecticut and CT Health Foundation and administered by CHDI. Two practices will be selected (up to $37,500 each) to test the feasibility of new practice services that could contribute to population health, health equity, and better integration of primary care services with community services.
Request for Letters of Intent (LOI): Funding for Pediatric Primary Care Practices "Transforming Pediatrics to Support Population Health"
The purpose of this grant program is to explore the feasibility and resources needed to integrate innovative, community-based, and evidence-based health promotion and prevention services into pediatric primary care. These grants provide an opportunity for pediatric primary care practices to better support the health and development of children by expanding their scope of work to include services that are not currently paid for in traditional fee for service reimbursement programs. Applicants are encouraged to consider a broad array of non-medical services that could be a part of their care and initiate, deepen, and/or expand health promotion and prevention services in collaboration with community partners.Project outcomes will inform and advance efforts by policymakers, providers, program staff, and advocates committed to health equity and payment reform to better address the needs of children and families.
