Partner Spotlight: How LifeBridge is Expanding Access to Care

News

CHDI has partnered with LifeBridge Community Services in Bridgeport since 2018 to implement Trauma-Focused Cognitive Behavioral Therapy (TF-CBT). Six LifeBridge clinicians have completed TF-CBT training with CHDI, and we continue to provide ongoing implementation support to their team as part of our ongoing work to expand access to evidence-based behavioral health treatments for children and youth in Connecticut.

Recently, LifeBridge shared some of the innovative strategies they've been using to make behavioral health care more accessible and connected across the Greater Bridgeport community with our TF-CBT provider community. We thought we'd share them here, too:

  • LifeBridge’s President and CEO and Senior Director of Behavioral Health met with local Department of Children and Families (DCF) offices and community organizations to share information about LifeBridge’s mental health services. These presentations strengthened partnerships, raised awareness, and expanded access to care, building a more connected and supported community.
  • The LifeBridge team has also embedded clinicians within local high schools and regional pediatric offices, creating seamless access points for youth and families.
  • They recently offered tours of their newly renovated space in Bridgeport to strengthen collaboration with community partners and have expanded both their clinical team and client capacity.
  • They maintain close communication with referral partners to ensure coordinated care and timely support for clients.
  • To simplify access, all agency materials now include a QR code allowing providers and families to make direct referrals with a single tap —scheduling intakes within 24 to 48 hours.
  • Outreach uses welcoming, non-stigmatizing language, and digital referral forms are linked to social media channels, helping partners and families easily connect with services and resources when they’re needed most.
  • Recognizing that the 3–5 p.m. time slot is most convenient for families after school, leadership recently extended hours of operation two days per week to better accommodate client schedules.

Kudos to the LifeBridge team, and thank you for sharing these strategies with our community!
