Connect4Families Online Toolkit Now Available

Connect4Families Venn Diagram.pngThe new Connect4Families Toolkit helps families, schools, and pediatric providers collaborate and connect children to behavioral health services and supports in Connecticut.

The Connect4Families Toolkit simplifies access to behavioral health services 
and facilitates collaboration among families, schools, and pediatric providers. By working together, families and providers can identify and manage behavioral health issues more quickly,
comprehensively, and consistently across settings, resulting in better outcomes for youth.
 
The toolkit includes a comprehensive service directory to facilitate referrals, along with best practices, tips, strategies, customizable communication forms, and more.
 
Toolkit sections include:

  2. The Why, What, How, and When of Effective Collaboration
  3. Electronic Tools to Support Collaboration
  4. Confidentiality Protections and Consent to Share
  5. Collaboration Tips from Parents, Youth, and Providers
  6. The Evolving Coding and Billing Landscape for Providers

 About the Connect4Families Toolkit

The Connect4Families Toolkit was produced by the Connecting to Care initiative’s Family Care Connections (FCC) Workgroup as part of a grant to promote communication and collaboration among child-serving sectors and support families and youth as full partners in driving the transformation of Connecticut’s system of care.
 
The toolkit content was developed by nationally recognized integrated care expert Barbara Ward-Zimmerman, Ph.D., with support from FCC Workgroup partners, including The Child Health and Development Institute (CHDI)Carelon Behavioral HealthThe Consultation Center at Yale, and FAVOR, Inc.
 
CHDI serves as the Coordinating Center for the Connecting to Care Initiative.
